Are you ready for the best spa party ever? We sure are! In order to give you the best experience possible, we need information from you and your group members. We ask that the host enter all guest information for completion.
Please reach out to your group and confirm their requested services and contact info and enter in the form below. From each guest you will need the following information:
Name & Contact Information: This information is necessary to add each guest to the reservation. Everyone must provide a unique phone number so that we can send the required Medical Intake Form and COVID waiver the day of your event. We are not able to send multiple Intake Forms to the same number.
Requested Services: Enter each guest’s top 2 desired services as well as a backup service using the drop down menu. We ask for a backup service to ensure we can provide a good flow of services with minimal to no wait times. Currently facials and pedicures are available, but limited.
Should you need to add, change or cancel anyone after you submit the form, just send us an email at relax@lontisdayspa.com
Additional Considerations: Though we do our best to accommodate spa party service requests, some options may be limited due to space and staffing. We are also not able to accommodate all guests being in the same room for all services in most cases. For some groups we may need to utilize our couples massage room, or we can upon request. If you have specific expectations or questions about this please let our team know in advance.
